Athletics Registration

The Bordentown Regional School District processes all of our physical paperwork online, please adhere to the dates below. All information must be submitted through our website. Please scroll to the bottom of the page for Physical Due Date Timelines. Below are the instructions needed to begin the process:

2.  Please download your Pre-Participation Physical Form and take this form to the physical appointment for the parent and physician to compete. Once complete, bring in the original physical forms into the nurse. 
3. Click onto the Athletic Team Registration Icon to begin the registration process
