Information for New/Transferring Students

Welcome to BRHS!!!!  Transferring to a new school can be anxious for both the student and family.  Below you will find some helpful medical information that is required when registering here at BRHS.

Health Documents Needed for New/Transferring Students

If transferring into school from another school within New Jersey students need:

1.  Proof of up to date immunizations

2.  Proof of recent physical exam (within one year)

If transferring into school from another school outside of New Jersey need:

1.  Proof of up to date immunizations

2.  Medical exam within 30 days of moving into the state of NJ.

If you do not have medical insurance or a primary care physician, please contact me so that I can assist you in completing all required paperwork as soon as possible.

** When taking your child to their yearly medical exam, it is good practice to bring a NJ Pre-Participation Physical Evaluation Form, so that your primary physician can complete.  This is good to have on file in case your student decides to participate in interscholastic sports team at the High School. **  This form can be found on the right side of this page.
