Welcome to the Bordentown Regional High School Nurse website.
If your student requires any medication to be given during the school day, you MUST have proper documentation in order for the medication to be given during school hours. This includes all prescription drugs and over-the-counter medications. Students are NOT permitted to carry any medications, both prescribed or over-the-counter, during school hours. All BRSD schools are now permitted to administer both Tylenol and Advil to students as long as we have parent permission. Therefore, these medications should remain home and not in school.
- Medication form must filled out by the physician and signed by the parent/guardian.
- Medication must be brought into school by an adult. If it is a prescription medication, it must be brought to school in an original medicine bottle with pharmacy label on it.
Requires the completion of the New Jersey State mandated Asthma Action Plan. You may find the form to the right of this page. Please review and have the form completely filled out (both sides) and return it to me as soon as possible. Students are permitted to self-carry and medicate for their asthma as long as there is an Asthma Action Plan on file in my office.
Life Threatening Allergies
If your student has any life threatening allergies and requires the use of an Epi-Pen, please contact the nurse so we can provide a safe learning environment for your child. If your student has been prescribed medication by his/her physician for their allergies, please see below for the appropriate form to be completed by both your physician and parent so the medication may be carried and given while your student is in school.
***One note for all medication forms - these forms are only good for one year. Therefore, they need to be renewed and sent in to school each school year. This is a State of New Jersey mandate. If you have any questions, please do not hesitate to contact me at any time.
