Sports Physicals

Dear Parent/Guardian,






The Bordentown Regional School District is excited to announce that we will begin processing all of our physical paperwork on line.  All information submitted will be done through our website.  We understand that like any change, there will be positives and negatives.  It is our goal to help you work through this process as efficiently as possible. We welcome your feedback on the new process and truly believe that this new process will be what is best for our students, their families, as well as the staff at BRSD.  The link for your online registration can be found on the high schools main page under the athletics page.


Below is a calendar of when all sports physicals are due as well as sports offered here at BRHS.

Sports physicals are good one year from the date of the examination.  Physicals must be completed and signed on the New Jersey Department of Education Annual Athletic Pre-Participation Physical Examination Form.  Physical forms are available on our school athletic website (  Please understand that for each sport a student wishes to try out for they must complete the permission form granting them permission to compete in that sport. They do not require a physical for each season, but for each season they must complete the permission form. 


All completed physical forms must be turned into Mrs. Redler, school nurse.   It is very important to complete this obligation or participation will not be allowed.  Should you have any questions regarding physicals please contact our school nurse Mrs. Redler via email  or Mr. Joseph Sprague at 609-298-0025 ext. 1100.


** Note - please understand that all sports physicals must still be sent to our school physician for clearance.  This process does take additional time once your physical is turned in.  Therefore, try to have your physical paperwork turned in by the due date or as soon as possible to avoid a delay in try outs/practices.



Also - if your student needs to carry any medication (ie. inhaler, epi-pen or other) the appropriate form must be filled out and attached with your physical to keep on file.  No physicals will be accepted without these forms attached.  Please contact me if you have any questions.  The forms you may need are found at the bottom of this page or on my main page.




You must have a current sports physical on file to participate in any Interscholastic Sport
